I understand that COVID 19 is spread by respiratory droplets and I understand that to enter the office I will need to wear a proper mask over my mouth and nose for the duration of the visit.
I understand that I should not come if I am ill, or have experienced any of the following symptoms within 14 days of my appointment: loss of sense of taste or smell, cough, unexplained digestive upset or skin rashes, shortness of breath, fever, chills or unexplained muscle aches.
I understand that office policies have changed which will impact my appointment, including but not limited to remaining in my vehicle until instructed, and abbreviated intakes in order to minimize contact.
